As this is my first time I could not rate it any higher. We filled it in a gave to our lawyer who didn't have to make too many changes/ The main one was Sales added in front of each word Agent as it had a different meaning to the one we needed.
The heading summed up what we were looking for.
Our legal fees were greatly reduced.
