About this document
Tenants should use this form when giving notice to the lessor/agent that they wish to vacate the premises. It may only be given in accordance with the Residential Tenancies and Rooming Accommodation Act.
Whilst this form is for the tenant to complete, it is a good idea for agents and lessors to keep a spare, blank copy of the form to give to tenants who contact them asking how they can initiate the ending the tenancy agreement.
