This is the official form for tenants to complete to record the condition of the premises at the end of the tenancy. As a lessor or agent, it is a good idea to keep a blank copy of this form to send to tenants at the end of the agreement.

The tenant completes the form and sends a signed copy to lessor / agent, who double checks the tenant’s entries, then counter signs the form and returns it to the tenant.
