These are Enduring Power of Attorney documents for use in Victoria under section 5A of the Medical Treatment Act, 1988 and Part XIA of the Instruments Act, 1958. We also have EPoA documents for other States.
An enduring power of attorney is used to allow someone else to look after your affairs when you are no longer able to do so yourself. Most people need one, because medical science is better at preserving our bodies than our brains.
These documents are suitable for:
- anyone making an EPA, and;
- anyone taking on the role of attorney
Which to choose?
Most EpoA documents cater only for financial affairs. The documents used to cater for medical and welfare treatment are not usually called EPoAs. In Victoria, they are.
First, choose whether you want a medical EpoA or a financial EpoA. Then choose whether you want one or two attorneys if you chose financial, or whether you want an alternative, back up attorney if you chose medial.
Full guidance notes are included.
