This is an Enduring Power of Attorney document for use in New South Wales under the Powers of Attorney Act 2003. We also have EPoA documents for other States.
An enduring power of attorney is used to allow someone else to look after your affairs when you are no longer able to do so yourself. Most people need one, because medical science is better at preserving our bodies than our brains.

- anyone making an EPA, and;
- anyone taking on the role of attorney
